\n3.\tThe applicant must be a Burmese national who is at least 18 years old and a Buddhist male. Both parents of the applicant must be Buddhist Burmese citizens.
\n4.\tIf you are a departmental employee, the application must include an official recommendation with the relevant department\u2019s office seal.
\n5.\tYou must purchase the application form by presenting the original National Registration Card or Citizenship Verification Card. Along with the application form, you must submit (a) a copy of the National Registration Card or Citizenship Verification Card (b) a copy of the household register.
\n6.\tThe application must be completed in Burmese only.
\n7.\tThe permit must be collected in person within 2 months from the date of application. No representative is allowed to collect the permit. If you do not collect it within (2) months, you will have to reapply.
